Fall Ball Documents/Check-in

Saturday Morning Check-in:
Prior to your first game on Saturday, please bring 1 copy of your team roster and any guest player roster approvals, along with all player cards to the nearest field marshal tent for check-in.  Your player cards will be zip-tied together to indicate they have been checked and are valid for the tournament. Please make sure your cards are laminated and hole-punched.

Pre-Tournament Registration

Coaches and Managers,

All team pre-registration will be done online through GotSport.  All documents must be uploaded by September 15, 2022. Please follow the directions at the link :

Team Check-In:
All teams:
a.  Official state approved roster (From your state association/US Club/SAY/Rec Organization. Please cross off any players not attending the tournament.  If you have a pool team, please cross off all players not playing for the specific team before uploading).
b.  Approved Player & Coach cards (Front & back if player/team information is on both sides).
c.  Permission to travel (if your team is carded outside of Ohio, Not required if roster is US Club Soccer). Teams from outside Ohio are required to provide an approved travel permit from their respective state association.  This includes both Kentucky and Indiana teams participating in Cincinnati area league play if carded by KY or IN.
d.  Guest Player Roster (from your state association). The maximum number of guest players for U8-U12 teams is (4), U13-U19 teams is (5). 
– For Ohio South Guest Player Forms, please use the link on the osysa.com website.
e.  Guest Player Cards (Front & back if player/team information is on both sides).
f. United Cup Liability Release & Compliance Form (download below). Only one (1) form per team will be accepted.  Please list
players in order as they appear on the roster. Guest players must be included.

For all teams outside of Ohio (all required):

g.  One copy of the state of Ohio coaches concussion training certificate for each coach (Ohio law).  Please 
visit https://www.nfhslearn.com/courses/61129/concussion-in-sports  Each coach will need to complete this course and then upload
the certificate. Certificates expire after 3 years. Must be a current certificate.

Medical release for players do not need to be uploaded, however we request that you have them on-hand at the tournament.
